12.2. Acalculous cholecystitis: The gallbladder is seen as an anechoic structure next to the liver; no stone is visible. The wall, especially towards the liver, is thick and hypoechoic due to inflammatory oedema in the gallbladder wall. As the patient complained of regional pain on palpation and also of fever, cholecystitis is very probable. The ultrasound can be used to guide palpation to the diseased gallbladder with precision. Note that the sonographic image alone is ambiguous. Other diseases (cirrosis with portal hypertension, congestive heart failure) may cause wall oedema.
